Preclinical development of new anticancer strategies based on the telomeric G quadruplex ligands in colorectal cancer advanced models

Abstract
In Western countries colorectal cancer is the third leading cause of cancer death. The treatment of this disease involves both surgery and chemotherapy that consists of combinations of multiple drugs. Despite the improvement in chemoterapy strategies, the phenomenon of drug resistance can occurs, and often is caused by the mutated KRAS oncogene, in whose promoter is present a G-quadruplex forming sequence. For this reason, the study of new compounds directed against the activity of KRAS is of great interest. In this PhD thesis was studied the antitumor efficacy of the G-quadruplex ligand EMICORON alone and in combination with chemotherapy in advanced models of colorectal cancer. At first, we verified the efficacy of the EMICORON in combination with the topoisomerase I inhibitor (SN-38) in a 3D cell culture system, and confirmed the synergistic effect in a xenograft model of colorectal cancer. Then, we obtained three PDXs (Patient-Derived-Xenograft) models of colorectal cancer. In all three KRAS mutated models we verified the antitumor efficacy of EMICORON treatment. Furthermore, in one of the models we demonstrated the efficacy of the combination of EMICORON with one of the treatment regimens adopted in clinical treatment of colorectal cancer. Finally, to confirm the efficacy of the compound in the treatment of mutated KRAS tumors, we studied the molecular activity of EMICORON on KRAS gene expression.
Nei Paesi Occidentali il cancro del colon-retto rappresenta la terza causa di morte per tumore. Il trattamento di questa malattia prevede sia la chirurgia che la chemioterapia che consiste in trattamenti con combinazioni di più farmaci. I fenomeni di chemoresistenza che si verificano sono spesso causati dalla mutazione in KRAS. É infatti di grande interesse lo studio di nuovi composti diretti contro l’attività di questo oncogene, nel cui promotore può formarsi una struttura G-quadruplex. In questo lavoro di tesi di dottorato è stata studiata l’efficacia antitumorale del ligando G-quadruplex EMICORON da solo e in combinazione con la chemioterapia in modelli avanzati di cancro del colon retto. Innanzitutto, abbiamo dimostrato l’efficacia dell’EMICORON in combinazione con l’inibitore della topoisomerasi I (SN-38) in un sistema di coltura cellulare 3D, e abbiamo poi confermato il sinergismo anche in un modello xenograft di cancro di colon-retto. Dopodichè, abbiamo ottenuto tre modelli PDX (Patient-Derived-Xenograft) mutati in KRAS. In tutti e tre i modelli abbiamo verificato l’efficacia antitumorale del trattamento con EMICORON e, inoltre, in uno dei modelli abbiamo dimostrato l’efficacia dell’EMICORON in combinazione con uno dei regimi di trattamento adottati in clinica nel trattamento del cancro del colon-retto. Infine, per confermare l’efficacia del composto nel trattamento di tumori KRAS mutati, abbiamo studiato l’attività dell’EMICORON sull’espressione genica di KRAS.
